PRIVACY OF CHILDREN
We request parental use of this application when it comes to minor children under 13 years of age. We have enabled the application to be completed by someone other than the person, such as a parent, and request the relationship of the party completing screening.
Parents should actively review the application and ensure minors understand the screening process. For all children under 13 years of age, this is required, and it is recommended for young adults under 18.
We hold safety and privacy as a priority.
WHAT INFORMATION IS GATHERED
Requested data:
Name (Last and First) - This is your identifier so we know who you are.
Email - This is used to send automated reminders for the on-line wellness screening at the facility.
Phone number - In the event of an outbreak, we may need to privately and instantly reach you to advise of exposure. We do not call or text you for any other reason. WE WILL NEVER ASK FOR OTHER PRIVATE INFORMATION.
Date and time - This is used for tracking of your entry to the facility.
Temperature assessment - Status checking that you are well and your body temperature is not more than 99.4.
Wellness screening - We ask these questions based on CDC guidelines for public spaces. We want to keep you and those around you in a safe environment.
The data gathered does depend on the facility, check with your facility for specifics.

WELLNESS SCREENING ASSESSMENT
Our wellness surveys are based on the CDC recommendations and other factors pertinent to entry to certain facilities. The answers to your questions are held private until such time as the CDC requests the details.
The email used when you first accepted this application, is linked to the daily reminder screening survey and only that email will receive reminders.
CDC COMPLIANCE
CDC has provided details regarding tracing and can be found at the link below. The information we gather will only be issued to the CDC at their request in the event of an outbreak. Each facility has protocols for Covid-19 breakouts and you should request policies from the facility you entered on how you will be contacted in the event of a breakout.

WHO CAN ACCESS DATA?
Your information will never be sold or provided to a third party other than as stated upon a formal request.
Information is available to the facility you entered and used to meet safety protocols.
Safe Business Solutions maintains access to data to support customers who rely on the data to meet their safety protocols. From time to time updates are made, reports are generated for the facility and we may review data integrity from time to time.
HOW LONG IS THE DATA HELD?
In the case of temperature assessment, 24 hours or 30 days depending on the facility.
In the case of wellness screening, 14 days to 30 days depending on the facility.
After these periods of time, all data is deleted from any storage location and irretrievable.
